With guidance from the Director of Population Health Management, the RN Population Health Coach works in collaboration and continuous partnership with patients, their family/caregiver(s), providers, clinics, specialty providers, staff and other healthcare professionals to provide individualized care management across the continuum of care for patients within the clinically integrated network (CIN), Arkansas Health Network. The RN Population Health Coach is responsible for coordinating a wide range of self-management support and disease registry activities in collaboration with healthcare team members to ensure safe, quality and cost effective care.HIPAA SECURITY COMPLIANCE:Security Access: High Incumbent has access to restricted or confidential patient informationand must comply with the terms of the CIN and delivery system Security Policies as it applies totheir job role.MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:Education: Associate Degree in Nursing with RN license plus 5 years relevant experience required; Bachelor of Science in Nursing or Master of Science in Nursing preferredLicensure/Certification: A current unencumbered RN license to practice in the State of Arkansas is required. Certification as Healthcare Coach or obtained within two years of hire. Certification in case management (CCM), public health and/or community health preferred.Experience: Five years of clinical, community, public health, case management, or health coaching experience required. Nursing education experience preferred. Experience working with Medicare and Medicaid populations and related social/economic issues preferred; post-acute or wound care experience preferred. Skills: Must have strong organizational (time management) skills, strong interpersonal skillsand the ability to handle multiple priorities. Knowledge of and practical use of good business English, spelling, arithmetic, practices and the ability to communicate effectively using written and verbal skills. Proficient in email communications and internet usage along with basic use of Microsoft Excel and Word. Knowledge of information technology to evaluate care effectiveness (care process, outcomes and cost). Basic Life Support (BLS) for the Healthcare Provider certified or obtained by the end of the orientation period (approximately six (6) weeks). Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire. Ability to work autonomously within matrix environment without direct supervision or support. Demonstrates a wide theory base and sound clinical skills to function as a nurse generalist.ESSENTIAL FUNCTIONS:1. Uses assessment skills and appropriate risk assessment tools to identify patients with actual or potential health care needs that would require care management intervention2. Works with at risk patients and families on self-management support including:a. Following evidence-based care pathwaysb. Coordinating care across multiple provider sitesc. Working with patients to create a plan of care for health behavior change:i. Assessing and working on the patients readiness to change, the importance of change, and confidence in ability to changeii. Helping the patient to identify and overcome barriersiii. Setting short and long-term goals for self-management of chronic diseaseiv. Referring to appropriate services when applicabled. Performing individual needs assessment, care plan design, documentation and implementation, and evaluation of outcomese. Providing needed patient education regarding specific health care skills and general disease conceptsf. Proactively identifying solutions to promote low acuity utilization before complications become high priorityg. Facilitating collaboration, communication and coordination among all responsible parties of an individuals multidisciplinary healthcare team striving to eliminate fragmentation, duplication or gaps in care3. Actively participates in quality activities:a. Assessing and collaborating with clinic managers and directors for CIN practices on strategies to achieve individual clinic level goals such as quality and efficiencyb. Communicating and coordinating with health care team in the development of tools for optimal patient outcomes and report findingsc. Meets on a regular basis with other outpatient population health team, as coordinated by CIN Leadership, for information sharing and continuing education activities4. Accountability for results:a. Understands and self-manages to support CIN-level success goals, including improvements in quality, cost of care and member experience for CINs populationb. Identifies opportunities for improvement (at individual, clinic and CIN levels) and actively works with healthcare and CIN team to correct or improve results5. Behaviors:a.
