team at Maple Lawn Senior Care in Fulda, MN . We offer a

Position Type: Full-Time Salary Range: $45 - $55 per hour, based on
experience. Bonus: $5,000 Location: 400 NE 7th St, Fulda, MN 56131
Responsibilities of the Physical Therapist: Assessment & Diagnosis:
Evaluate patients' physical conditions, mobility, and medical
histories to create tailored treatment plans. Plan Development &
Implementation: Design and execute rehabilitation programs aimed at
enhancing patients' functional abilities. Hands-on Therapy: Conduct
manual therapy, therapeutic exercises, and utilize modalities such
as ultrasound and electrical stimulation as required. Patient
Education & Support: Educate patients, caregivers, and staff on
necessary clinical topics, including pain management strategies and
fall prevention. Instruct patients and families on exercises and
injury prevention techniques. Progress Monitoring & Plan
Adjustment: Track patient outcomes, document progress, and modify
treatment plans as necessary. Collaboration with Healthcare Teams:
Partner with physicians, nurses, and other therapists to ensure
holistic patient care. Qualifications for the Physical Therapist:
Graduation from an accredited Physical Therapy Program and
possession of a bachelor's degree. Current licensure as a Physical
Therapist in Minnesota. Strong understanding of state, federal, and
local regulations related to Home Health care. Able to perform
essential job functions with or without reasonable accommodations.
Proficient in English, both spoken and written, with adequate
communication skills to fulfill job requirements. Benefits Offered:
