In the role of Physical Therapist (PT), you will assess the
physical capabilities of residents and create tailored treatment
plans aimed at helping them regain or sustain mobility,
functionality, and independence. Your responsibilities will include
delivering hands-on therapy, educating patients and their families,
and collaborating with interdisciplinary teams to achieve the best
possible outcomes. Primary Responsibilities Perform thorough
assessments of residents’ physical abilities and mobility using
standardized evaluation tools. Design and execute evidence-based
treatment plans to enhance strength, balance, coordination, and
range of motion. Administer therapeutic interventions, including
exercise regimens, gait training, and the use of assistive devices.
Provide education to residents, families, and staff on safe
mobility practices and home exercise routines. Accurately document
evaluations, progress notes, and discharge summaries in compliance
with regulatory and organizational standards. Work in conjunction
with occupational therapists, speech-language pathologists, nurses,
and other team members to ensure coordinated care. Adhere to all
relevant state and federal regulations. Qualifications Valid
licensure as a Physical Therapist (PT) in the state where you
practice. Graduation from an accredited Physical Therapy program.
Experience in long-term care, skilled nursing, or rehabilitation
settings is preferred. Excellent communication, documentation, and
