Responsibilities:
Provide care aligned with a personalized Plan of Care
Monitor and track changes in the client's conditions and keep Director of Nursing informed as well as other necessary personnel
Provide services requiring substantial and specialized nursing skill
Proactively update plan of care
Document health changes and concerns
Requirements:
Current LPN license in the state of practice required
Current CPR certification required
Must know how to draw blood
High school diploma or equivalent required
Basic computer skills
